Who MUST Enroll?
Most people with an HFS or All Kids medical card must join.
Who CANNOT Enroll?
- People who receive Medicare.
- Kids under age 21 who get Supplemental Security Income (SSI).
- Children in foster care and children who get Subsidized Guardianship or Adoption Assistance from DCFS (Department of Children and Family Services).
- Kids under age 21 who are blind or who have a disability.
- People who reside in nursing facilities.
- American Indians and Alaska Natives.
- People with Spend-down.
- Refugees.
- People who get Home and Community-Based Services like the Community Care Program, the Home Services Program, or Community Services for Persons with developmental disabilities.
- People residing in Community Integrated Living Arrangements (CILAs).
- People enrolled in the following programs:
- Illinois Healthy Women
- All Kids Rebate and FamilyCare Rebate
- Illinois Cares Rx (SeniorCare/Circuit Breaker)
- Transitional Assistance, age 19 or older
- Emergency Medical Only
- Hospice
- Renal and Hemophilia programs
- Children under age 21 whose care is managed by the Division of Specialized Care for Children (DSCC) of the University of Illinois at Chicago
How Do I Enroll?
1. Pick a health plan
Most people can pick Illinois Health Connect - (available in all counties), Family Health Network - (available in Cook County only), Harmony Health Plan - (available in Cook, Jackson, Kane, Madison, Perry, Randolph, St. Clair, Washington and Williamson County) or Meridian Health Plan - (available in Adams, Brown, Henry, Mercer, Perry, Pike and Rock Island County).
2. Pick a Primary Care Provider (PCP)
To pick the doctor that is best for you and your family, think about where you and your family get care now.
- Do you want to stay with that doctor?
- Do you know another doctor or clinic that you like?
- You do not have to choose the same doctor for all members of your family.
- Look for a doctor online

- Or call your health plan
3. Enroll
There are four ways to enroll. Pick the one that works best for you.
- PHONE: Call the ICEB Helpline at 1-877-912-8880 (TTY: 1-866-565-8576). You can call the Helpline if have any questions or need help. The call is free.
- FACE-TO-FACE: Call the ICEB Helpline at 1-877-912-8880 (TTY: 1-866-565-8576) to schedule an appointment at a local DHS office. You can also attend one of the community events.
- MAIL: Fill out the enrollment form you received in your packet. Mail the form back to the Illinois Client Enrollment Broker in the envelope provided.

After you join, you will get confirmation from your health plan. At that time, you can begin making appointments to see your doctor. When you need health care services, you will call your doctor first, unless it is an emergency. If you need a referral to see a specialist, your doctor will give you one. Always seeing the same doctor will mean better care for you and your family.
